New My Health – Living Well with Long-Term Conditions service builds on Long Covid success to support more patients with ongoing symptoms
NHS Greater Glasgow and Clyde (NHSGGC) has launched My Health – Living Well with Long-Term Conditions, an expanded service designed to support people living with persistent symptoms that affect their daily lives, wellbeing and independence.
The new service builds on the success of NHSGGC’s Long Covid Service, extending specialist support to a wider group of people living with long-term and post-viral conditions. The service has evolved in response to growing clinical understanding of the similarities that can exist across a range of conditions that cause ongoing symptoms and impact quality of life.
The My Health – Living Well with Long-Term Conditions service is designed to support adults experiencing symptoms for 12 weeks or more that affect their daily living, work, and wellbeing. The service will support people living with Long Covid, Myalgic Encephalomyelitis (ME), Chronic Fatigue Syndrome (CFS) and post-viral fatigue, while also providing support for selected patients with other symptom presentations that significantly affect wellbeing and function.
The service is led by experienced Allied Health Professionals (AHPs), including occupational therapists, physiotherapists and support staff, who work closely with primary care teams and wider healthcare services to ensure patients receive the right support at the right time.
Referrals to the service can be made by GPs and the GP Multi disciplinary Team where specialist support is considered beneficial.

That experience has improved our understanding of the similarities that can exist across a range of long-term and post-viral conditions, and the challenges these can present for patients in their daily lives.
“My Health – Living Well with Long-Term Conditions builds on that learning, allowing us to extend specialist support to more people who are experiencing persistent symptoms that impact their health, wellbeing and independence.
“Importantly, people currently using our Long Covid Service will continue to receive the same high-quality support, while more people will now be able to benefit from the knowledge, expertise and care developed by our teams in recent years.
“This service brings together expert clinical knowledge and a person-centred approach to care, ensuring patients can access the right support, information and resources to help them manage their condition and live as well as possible.”
Alongside the launch of the service, NHSGGC has also launched My Health, a digital resource which provides trusted online information, practical advice and self-management support for everyone, but is particularly helpful for people with long-term conditions. This free resource offers information on health and wellbeing, preparing for healthcare appointments and managing common symptoms, helping people feel more informed, confident and supported in their healthcare journey.
My Health is designed to complement face-to-face care by helping people prepare for appointments, interventions and treatment, while providing reliable information they can access whenever they need it.
People looking for support can access a range of self-management resources through My Health, while those who may require specialist intervention can discuss referral options with their GP or Primary Care team.
